📍 Step 2: Download & Print
You’ll get a PDF packet with instructions, legal forms, and often a checklist. Print it or save it to share with your provider or healthcare proxy.
📍 Step 3: Fill it Out & Attach It to Your Planner
Once completed, include a copy in your Life and Death Planner. Also share it with:
Your primary care provider
Your healthcare proxy
Any close family or friends involved in your care
